e-learning courseware followed by an hour's face to face time, finally followed up over several weeks with line manager weekly meetings

More recently blended learning has come to embrace the huge opportunity that informal learning offers: basically the learning we achieve by being social in social interaction, whether it's in work or out of work, in-the-flesh or online.
The bottom line across all of this is that people are social animals, and learning can't all be about engaging with technology - even when its delivered on a Nintendo DS or the iPhone (even though that is very cool!).
A mix of formal (technology, classroom, outside classroom) and informal (technology, outside classroom) activity is now not just how we should see blended learning, but how we should see organisational learning today and in the future.
We don't have a 'one-size-fits-all' approach when it comes to blended learning - in fact we think it would be a pretty big waste of such a flexible idea to do so.
